Acceptable proof of address:

Recent Utility Bill (Gas, electric, landline phone, water, cable, or internet bill from the last 90 days showing the service address)

Current Mortgage or Lease Agreement (for a current Chicago residence)

Deed (showing current Chicago property ownership)

MediPlan / Medicaid Card or Illinois Department of Public Aid Card

Voter Registration Card (with a current Chicago address)

Stamped U.S. Post Office Change of Address Form (confirming Chicago residency)

Illinois State Aid or Social Security Check (mailed to a Chicago address)

Court Documents

Other Official Identification Card issued by a federal, state, or foreign government consulate agency (such as a Matrícula Consular)

Additionally, you will need to bring a Parent ID, your student's Birth Certificate, and their Medical Physical.
